Issue

An Organisation uncovered a potential Conflict of Interest involving a Senior Leadership member. They were still determining if the actions had violated their Conflict of Interest policy and wanted to understand what action should be taken against the individual.

Request

We were asked to help formally investigate the violation and document the findings in a manner that supports any disciplinary action required.

Action

We designed an investigation plan, reviewed all aspects of the incident, reviewed communications over email based on keyword searches and cross-referencing programmatic information, reviewed the Organisation’s Conflict of Interest and Gift policy and interviewed witnesses and the subject.

Outcome

We provided an investigation report establishing the Conflict of Interest policy had been violated. The Organisation used the report to take decisive disciplinary action against the Leadership member, protecting their reputation internally and externally. We recommended urgent amendments to close gaps in the Conflict of Interest and Gift policy that our investigation had identified.
